What Do I Need to File?
All employers offering self-insured/level-funded minimum essential health coverage to their employees must file either a 1095-B or 1095-C Form.
Step 1: Are you an ALE?
An Applicable Large Employer (ALE) is an employer with 50 or more full-time employees (including full-time equivalents) on average during the previous calendar year.

If you are an ALE you will need to file a 1095-C form. Our services are only available for 1095-B forms, If you need assistance in filing a 1095-C form, we recommend ACAPrime as a preferred vendor.
If you are not an ALE, you must file a 1095-B form. Continue with the following steps and 1095b-ezfile will help you file your forms securely and electronically.
Step 2: Download the Excel Workbook
Step 3: Complete the Excel Workbook
Fill in the employer data, employee listing and dependent sections. Instructions on how to complete the spreadsheet can be found on tab one of the workbook. Once completed save a copy to your desktop.
Step 4: Upload Workbook & Submit Payment
Click the link below to securely upload the workbook and submit final payment for processing. Once submitted our team will begin the review and auditing process and follow up with you for any missing information and/or next steps prior to filing with the IRS.
